Registration Process

I, as parent/guardian of the above named swimmer(s), waive, release, absolve, indemnify and agree to save harmless the SWAT, its members, executive, coaches and staff from any liability and any and all rights claiming damages which I, or the above noted swimmer(s) may have against SWAT for injuries sustained while participating in the SWAT program and activities related thereto.
